His retreat, they add, could end decades in which the agency directed the management of smallpox virus remnants in an American-held cache.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Health experts say discontinuation of the W.H.O.\u2019s oversight threatens to damage precautions against the virus leaking into the world, and to disrupt research on countermeasures against the lethal disease. They add that it could also raise fears among allies and adversaries that the United States, under a veil of secrecy, might weaponize the smallpox virus.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">\u201cI\u2019ve been in that lab,\u201d said <a class=\"css-yywogo\" href=\"https:\/\/resolvetosavelives.org\/about\/team\/tom-frieden\/\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">Thomas R. Frieden<\/a>, a former director of the Centers for Disease Control and Prevention in Atlanta, where the American cache resides. \u201cImagine a submarine inside a building and the people walking around in spacesuits. It looks like something out of a movie.\u201d To reduce smallpox risks and misperceptions, Dr. Frieden added, \u201cwe need to open ourselves up to inspection.\u201d<\/p>\n<p class=\"css-at9mc1 evys1bk0\">On Monday, <a class=\"css-yywogo\" href=\"https:\/\/geiselmed.dartmouth.edu\/faculty\/facultydb\/view.php\/?uid=7588\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">Daniel R. Lucey<\/a>, a Dartmouth medical professor, <a class=\"css-yywogo\" href=\"https:\/\/www.idsociety.org\/science-speaks-blog\/2025\/u.s.-exit-from-who-potential-impacts-for-smallpox-virus-biosafety#\/+\/0\/publishedDate_na_dt\/desc\/\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">posted an article<\/a> on the blog of the Infectious Diseases Society of America warning that Mr. Trump\u2019s W.H.O. exit could imperil \u201csmallpox virus storage, experiments, reporting and inspections.\u201d<\/p>\n<\/div>\n<aside class=\"css-ew4tgv\" aria-label=\"companion column\"\/><\/div>\n<div data-testid=\"companionColumn-1\">\n<div class=\"css-53u6y8\">\n<p class=\"css-at9mc1 evys1bk0\">A half century ago, the W.H.O. purged <a class=\"css-yywogo\" href=\"https:\/\/www.who.int\/health-topics\/smallpox#tab=tab_1\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">the smallpox virus<\/a> from human populations after the scourge had killed people for thousands of years. Dr. Frieden called it \u201cone of the greatest accomplishments not just of medical science but global collaboration.\u201d<\/p>\n<p class=\"css-at9mc1 evys1bk0\">While the germ was eradicated in people, two repositories were preserved to allow study of the virus should it re-emerge: one in Atlanta, the other in Russia. To curb leaks, both caches are stored in special labs classified as <a class=\"css-yywogo\" href=\"https:\/\/www.nytimes.com\/2021\/11\/23\/magazine\/covid-lab-leak.html\" title=\"\">Biosafety Level 4<\/a>, the highest tier of protection.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">In recent years, the W.H.O., based in Geneva, <a class=\"css-yywogo\" href=\"https:\/\/www.who.int\/activities\/research-using-live-variola-virus\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">has ruled on<\/a> the safety and scientific merit of proposed studies of smallpox by both the C.D.C. and its Russian counterpart. It has the authority to <a class=\"css-yywogo\" href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC8454959\/\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">grant or refuse<\/a> permission despite its role being described publicly as advisory. The agency also <a class=\"css-yywogo\" href=\"https:\/\/www.who.int\/activities\/variola-virus-repository-safety-inspections\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">regularly inspects<\/a> the smallpox labs for safety lapses.<\/p>\n<\/div>\n<aside class=\"css-ew4tgv\" aria-label=\"companion column\"\/><\/div>\n<div data-testid=\"companionColumn-2\">\n<div class=\"css-53u6y8\">\n<p class=\"css-at9mc1 evys1bk0\">Health experts warn that Mr. Trump\u2019s exit from international oversight could end Washington\u2019s ability to scrutinize Moscow\u2019s smallpox cache. \u201cIf we want to inspect the Russian lab,\u201d Dr. Frieden said, \u201cwe need to be part of W.H.O.\u201d<\/p>\n<\/div>\n<aside class=\"css-ew4tgv\" aria-label=\"companion column\"\/><\/div>\n<div data-testid=\"companionColumn-3\">\n<div class=\"css-53u6y8\">\n<p class=\"css-at9mc1 evys1bk0\">Russia probably hides some smallpox virus for military use, according to <a class=\"css-yywogo\" href=\"https:\/\/www.nytimes.com\/1999\/06\/13\/world\/government-report-says-3-nations-hide-stocks-of-smallpox.html\" title=\"\">a federal intelligence assessment<\/a>. Health experts also warn that the American exit from the W.H.O. could prompt fears that the United States might weaponize the lethal virus.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">In his first term as president, Mr. Trump <a class=\"css-yywogo\" href=\"https:\/\/www.nytimes.com\/2020\/04\/07\/us\/politics\/coronavirus-trump-who.html\" title=\"\">lashed out<\/a> at the W.H.O. over its handling of the coronavirus pandemic and, in July 2020, <a class=\"css-yywogo\" href=\"https:\/\/www.nytimes.com\/2020\/07\/07\/us\/politics\/coronavirus-trump-who.html\" title=\"\">ordered<\/a> a withdrawal. But six months later, before the separation could be completed, former President Joseph R. Biden Jr. reversed Mr. Trump\u2019s decision on his first day in office.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">This time around, Mr. Trump came on stronger, issuing an <a class=\"css-yywogo\" href=\"https:\/\/www.whitehouse.gov\/presidential-actions\/2025\/01\/withdrawing-the-united-states-from-the-worldhealth-organization\/\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">executive order<\/a> within hours of taking office that gave notice of the U.S. withdrawal. He also told his administration to identify U.S. and global partners that could \u201cassume necessary activities\u201d that the W.H.O. had previously carried out.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">A week later, the C.D.C. <a class=\"css-yywogo\" href=\"https:\/\/apnews.com\/article\/cdc-who-trump-548cf18b1c409c7d22e17311ccdfe1f6\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">was ordered<\/a> to end all collaboration with the W.H.O.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">It is not clear if the W.H.O. and the C.D.C. have completely ended their smallpox cooperation. Their replies to email queries seemed contradictory, and the White House did not respond.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Christian Lindmeier, a W.H.O. spokesman, said officials at the agency were still seeking to clarify the implications of Mr. Trump\u2019s order. The agency, he added, \u201cis ready to work with the new U.S. Administration to sustain our vital collaboration.\u201d The next smallpox inspection, Mr. Lindmeier added, is scheduled for May 2026.<\/p>\n<\/div>\n<aside class=\"css-ew4tgv\" aria-label=\"companion column\"\/><\/div>\n<div data-testid=\"companionColumn-4\">\n<div class=\"css-53u6y8\">\n<p class=\"css-at9mc1 evys1bk0\">The C.D.C. is part of the department of Health and Human Services, whose communications director, Andrew G. Nixon, said only that it was complying with Mr. Trump\u2019s order for the U.S. to withdraw from the W.H.O.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Unlike most viruses, <a class=\"css-yywogo\" href=\"https:\/\/www.who.int\/health-topics\/smallpox#tab=tab_1\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">smallpox<\/a>, known as variola, is highly stable outside its host. It can long retain its power to infect, aiding its spread. Victims develop high fevers, deep rashes and oozing pustules. About a third die. In the 20th century alone, the virus is estimated to have <a class=\"css-yywogo\" href=\"https:\/\/www.nytimes.com\/1999\/06\/15\/science\/smallpox-the-once-and-future-scourge.html\" title=\"\">taken more lives<\/a> than all wars and other epidemics put together.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">In 1959, the W.H.O. resolved to eradicate the killer in a blitz of global vaccinations and quarantines. Little was achieved until Washington and Moscow in 1966 proposed a stronger effort. That year alone, the disease killed two million people. By 1977, the W.H.O. recorded its last case, defeating the deadly scourge.<\/p>\n<\/div>\n<aside class=\"css-ew4tgv\" aria-label=\"companion column\"\/><\/div>\n<div data-testid=\"companionColumn-5\">\n<div class=\"css-53u6y8\">\n<p class=\"css-at9mc1 evys1bk0\">Today, the American W.H.O. exit may raise the risk of the smallpox virus escaping confinement and reinfecting the world, some health experts say. They see that threat as implicit in how past W.H.O. inspections of the C.D.C. smallpox labs made <a class=\"css-yywogo\" href=\"https:\/\/www.who.int\/activities\/variola-virus-repository-safety-inspections\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">scores of recommendations<\/a> for safety and security upgrades.<\/p>\n<\/div>\n<aside class=\"css-ew4tgv\" aria-label=\"companion column\"\/><\/div>\n<div data-testid=\"companionColumn-6\">\n<div class=\"css-53u6y8\">\n<p class=\"css-at9mc1 evys1bk0\">The proposals included better security systems, training, risk assessments, inspections of pressure suits, accident investigations and monitoring of staff competency.<\/p>\n<p class=\"css-at9mc1 evys1bk0\"><a class=\"css-yywogo\" href=\"https:\/\/apps.ualberta.ca\/directory\/person\/devans\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">David H. Evans<\/a>, a virologist at the University of Alberta, has twice inspected the C.D.C. virus area and been a member of the W.H.O.\u2019s smallpox scientific advisory panel.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">While withdrawing from the W.H.O. may not increase the risk of a leak, he said, collaboration between the U.S. and the W.H.O. works quite effectively at improving research and lowering suspicions of illicit work.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">\u201cIt keeps people talking to each other,\u201d Dr. Evans said. \u201cIt gets back to the idea of transparency. It gives you some idea of what\u2019s going on.\u201d<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Last year, a panel of the National Academy of Sciences <a class=\"css-yywogo\" href=\"https:\/\/nap.nationalacademies.org\/catalog\/27652\/future-state-of-smallpox-medical-countermeasures\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">described the cooperative smallpox research<\/a> of the C.D.C. and the W.H.O. as \u201curgently needed.\u201d Looking beyond vaccines, the panel called for a new generation of antiviral drugs that would better fight smallpox in individuals already infected with the pathogen.<\/p>\n<\/div>\n<aside class=\"css-ew4tgv\" aria-label=\"companion column\"\/><\/div>\n<div data-testid=\"companionColumn-7\">\n<div class=\"css-53u6y8\">\n<p class=\"css-at9mc1 evys1bk0\">The W.H.O. research has had practical payoffs. Antiviral drugs developed by its smallpox program <a class=\"css-yywogo\" href=\"https:\/\/nap.nationalacademies.org\/catalog\/27652\/future-state-of-smallpox-medical-countermeasures\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">have been deployed<\/a> to fight Mpox, formerly known as monkeypox, <a class=\"css-yywogo\" href=\"https:\/\/www.nytimes.com\/2024\/08\/29\/well\/mpox-health-risks-vaccine.html\" title=\"\">which is spreading quickly<\/a> in parts of Africa, <a class=\"css-yywogo\" href=\"https:\/\/www.nytimes.com\/2024\/08\/23\/health\/mpox-vaccines-who-africa.html\" title=\"\">particularly in<\/a> the Democratic Republic of Congo.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">In 1996, W.H.O. members <a class=\"css-yywogo\" href=\"https:\/\/www.cidrap.umn.edu\/public-health\/experts-regulatory-issues-are-main-reason-keeping-smallpox-virus\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">agreed on a plan<\/a> to destroy the two remaining stocks of the smallpox virus. Now, increasingly, it aims to prepare for potential outbreaks rather than permanently extinguishing the virus.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">But those heightened levels of defensive planning could be undone by the American withdrawal, health experts warn. They fear more countries may leave the W.H.O., spurring new outbreaks.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Michael T. Osterholm, the director of the Center for Infectious Disease Research and Policy at the University of Minnesota, noted that Argentina, echoing Mr. Trump, <a class=\"css-yywogo\" href=\"https:\/\/www.nytimes.com\/2025\/02\/05\/world\/americas\/argentina-who-withdrawal-trump.html\" title=\"\">withdrew from the W.H.O. early this month<\/a>, and that <a class=\"css-yywogo\" href=\"https:\/\/www.politico.eu\/article\/hungary-viktor-orban-floats-who-withdrawal-after-trump-and-milei-exits\/\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">Hungary and Russia<\/a> have also explored the idea.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">\u201cGlobal health safety is effective only if we have global representation,\u201d Dr. Osterholm said. \u201cW.H.O. will do what it can.
