That included the fact that the &#8220;design and testing processes for TITAN did not adequately address many of the fundamental engineering principles that would be crucial for ensuring safety and reliability in such an inherently hazardous environment,&#8221; the report said.\u00a0<\/p>\n<p>The report also faulted\u00a0<span class=\"link\"><a href=\"https:\/\/www.cbsnews.com\/news\/titan-sub-implosion-window-on-the-path-of-failure-engineer-testimony\/\" target=\"_blank\" data-invalid-url-rewritten-http=\"\">OceanGate<\/a><\/span> for its continued use of the Titan sub even though the vessel had previously been damaged in &#8220;a series of incidents that likely compromised the integrity of the hull and other critical components of the submersible.&#8221;\u00a0<\/p>\n<p>The Coast Guard report says OceanGate had a &#8220;toxic&#8221; safety culture and corporate structure, and that its operational practices were &#8220;critically flawed.&#8221;<\/p>\n<p>&#8220;The two-year investigation has identified multiple contributing factors that led to this tragedy, providing valuable lessons learned to prevent a future occurrence,&#8221; Neubauer said.\u00a0<\/p>\n<p>Among those killed aboard the <span class=\"link\"><a href=\"https:\/\/www.cbsnews.com\/news\/titan-submersible-implosion-hearing-final-text-message\/\" target=\"_blank\" data-invalid-url-rewritten-http=\"\">doomed vessel<\/a><\/span> was <span class=\"link\"><a href=\"https:\/\/www.cbsnews.com\/news\/titanic-submersible-interview-transcript-with-oceangate-ceo-stockton-rush\/\" target=\"_blank\" data-invalid-url-rewritten-http=\"\">Stockton Rush<\/a><\/span>, co-founder of OceanGate, the Washington state company that owned the Titan.
