Drugs in the GLP-1 class, which includes Wegovy and Ozempic, have proved miraculous <a class=\"css-yywogo\" href=\"https:\/\/www.nytimes.com\/2023\/08\/17\/health\/weight-loss-drugs-obesity-ozempic-wegovy.html\" title=\"\">in treating weight loss<\/a> and <a class=\"css-yywogo\" href=\"https:\/\/www.nytimes.com\/2023\/08\/08\/health\/wegovy-obesity-drugs-heart-disease.html\" title=\"\">other diseases<\/a>. And some researchers hoped that the drugs could also help with some of the most difficult diseases to treat \u2014 those of the brain, like Parkinson\u2019s.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">But now, at least for Parkinson\u2019s, that hope seems dimmed. A rigorous study that randomly assigned Parkinson\u2019s patients to take exenatide, a relative of Ozempic, showed absolutely no benefit or slowing of the course of the degenerative disease after 96 weeks.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">And there were no effect on patient symptoms, no effect on brain scans, no subgroup that showed any benefit. No matter how the researchers sliced the data the results were the same.<\/p>\n<p class=\"css-at9mc1 evys1bk0\"><a class=\"css-yywogo\" href=\"https:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(24)02808-3\/fulltext\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">The study<\/a>, published Tuesday in The Lancet, is bad news for the half million Americans who have been diagnosed with Parkinson\u2019s disease. Symptoms include tremors, stiffness and difficulty with balance. Patients also may develop dementia. Treatments, including medications and deep brain stimulation, can help with symptoms. But no treatment has been shown to slow the disease\u2019s progress.<\/p>\n<\/div>\n<aside class=\"css-ew4tgv\" aria-label=\"companion column\"\/><\/div>\n<div data-testid=\"companionColumn-1\">\n<div class=\"css-53u6y8\">\n<p class=\"css-at9mc1 evys1bk0\">\u201cIt\u2019s hugely disappointing,\u201d said Dr. Thomas Foltynie of University College London, who led the trial. \u201cWe were expecting we would come through and we would get a positive result.\u201d<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Parkinson\u2019s experts shared his sentiment.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">\u201cThis is a sobering moment,\u201d said Dr. Michael S. Okun, a Parkinson\u2019s disease expert at the University of Florida and the national medical adviser for the Parkinson\u2019s Foundation. \u201cThis is a really well done study and it came up empty-handed.\u201d<\/p>\n<p class=\"css-at9mc1 evys1bk0\">The finding may have implications for researchers who are asking if the newer GLP-1 drugs <a class=\"css-yywogo\" href=\"https:\/\/www.nytimes.com\/2025\/01\/31\/well\/mind\/ozempic-alzheimers.html\" title=\"\">could help slow the course of Alzheimer\u2019s<\/a> or could prevent the disease.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">The new study involved 194 people with Parkinson\u2019s disease treated at six research hospitals in the U.K. The patients were randomly assigned to inject themselves once a week for 96 weeks with exenatide, a type 2 diabetes treatment made by AstraZeneca and sold under the brand name Byetta, or with a placebo. The trial was funded by Britain\u2019s National Institute for Health and Care Research with support for substudies from the charity Cure Parkinson\u2019s and the Van Andel Institute.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">The drug is in the same class as Ozempic and Wegovy and, like them, lowers blood sugar levels. All are so-called GLP-1 receptor agonists, commonly called GLP-1s. Exenatide is not as powerful in eliciting weight loss as the newer drugs, but experts say there is no reason to believe that the more recent drugs would perform differently in studies of brain disease.<\/p>\n<\/div>\n<aside class=\"css-ew4tgv\" aria-label=\"companion column\"\/><\/div>\n<div data-testid=\"companionColumn-2\">\n<div class=\"css-53u6y8\">\n<p class=\"css-at9mc1 evys1bk0\">The results, researchers said, are especially disappointing because there were suggestions that GLP-1 drugs might help Parkinson\u2019s patients.<\/p>\n<p class=\"css-at9mc1 evys1bk0\"><a class=\"css-yywogo\" href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/12183643\/\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">GLP-1 drugs protected neurons<\/a> from damage in laboratory studies and <a class=\"css-yywogo\" href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/30013201\/\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">in a study<\/a> with rats given a brain injury like that in Parkinson\u2019s disease.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">It began to seem that the results might also apply to patients.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">\u201cPeople started digging into claims databases,\u201d Dr. Okun said, explaining that researchers had examined large databases showing drugs that people took and their diagnoses. The researchers asked if patients who had taken GLP-1s might be less likely to get Parkinson\u2019s or, if they had it, would have a disease that progressed more slowly.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">The results were promising.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">They looked <a class=\"css-yywogo\" href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/39189078\/\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">at epidemiological studies<\/a>. They found that people with diabetes who took GLP-1s were less likely to have Parkinson\u2019s.<\/p>\n<p class=\"css-at9mc1 evys1bk0\"><a class=\"css-yywogo\" href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/24662192\/\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">Then two small studies suggested<\/a> that <a class=\"css-yywogo\" href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/28781108\/\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">exenatide might slow the progression<\/a> of some Parkinson\u2019s symptoms over a year\u2019s time.<\/p>\n<\/div>\n<aside class=\"css-ew4tgv\" aria-label=\"companion column\"\/><\/div>\n<div data-testid=\"companionColumn-3\">\n<div class=\"css-53u6y8\">\n<p class=\"css-at9mc1 evys1bk0\">Continuing the hints of progress, a larger but still preliminary study, <a class=\"css-yywogo\" href=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMoa2312323\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">published last year<\/a> in the New England Journal of Medicine, found that a GLP-1 that is no longer on the market \u2014 lixisenatide \u2014 appeared <a class=\"css-yywogo\" href=\"https:\/\/www.nytimes.com\/2024\/04\/03\/health\/parkinsons-ozempic-glp-1.html\" title=\"\">to slightly slow progress<\/a> of the disease over a year.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Dr. Okun, at the time, said that the result was \u201cnibbling at the edges of disease modification.\u201d<\/p>\n<p class=\"css-at9mc1 evys1bk0\">\u201cWhat we had last year was a one-year trial and a small signal,\u201d said Dr. David Standaert, a Parkinson\u2019s researcher at the University of Alabama at Birmingham. \u201cWhat would happen if you went longer? Well, this is longer and there\u2019s just not much here.\u201d<\/p>\n<p class=\"css-at9mc1 evys1bk0\">The problem with studying GLP-1s in Parkinson\u2019s disease, Dr. Standaert said, is that what exenatide is supposed to be doing in the brain is not clear.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">\u201cI wouldn\u2019t do another study like this unless you learn what is the target,\u201d Dr. Standaert said. \u201cWhat is the biochemistry you are trying to change in the brain?
