I want you to go to sleep for a little while.\u201d<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Ms. Rosa, 76, a memory care resident in assisted living, visits the nursery daily. She has had plenty of experience with babies.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">She was the oldest girl of eight children, so she handled lots of family responsibilities, she told Catherine Dolan, the facility\u2019s director of life enrichment, who was asking questions to help the memories flow. Later in life, Ms. Rosa worked in a bank and a store; the stories emerged as she cuddled the doll.<\/p>\n<\/div>\n<aside class=\"css-ew4tgv\" aria-label=\"companion column\"\/><\/div>\n<div data-testid=\"companionColumn-1\">\n<div class=\"css-53u6y8\">\n<p class=\"css-at9mc1 evys1bk0\">No actual babies live in this immersive environment, where the fragrance blend includes a talcum scent. Just as no actual sales were taking place at the store down the corridor, another new RiverSpring undertaking.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Amid its wooden shelves of clothing, accessories and tchotchkes, the sales clerks were, like Ms. Dolan, staff members trained to interact effectively with residents with dementia.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">\u201cGreat choice,\u201d said the cheerful cashier \u2014 Andre Ally, the engagement coordinator \u2014 to a 91-year-old who had selected a plaid muffler. \u201cPerfect for this weather.\u201d<\/p>\n<p class=\"css-at9mc1 evys1bk0\">The shopper handed over a plastic card that residents had been issued, which had no monetary value, and headed out with his walker, pleased about his new scarf. \u201cIt\u2019s very warm,\u201d he said. \u201cAnd a nice size, so you can wear it with any coat.\u201d<\/p>\n<\/div>\n<aside class=\"css-ew4tgv\" aria-label=\"companion column\"\/><\/div>\n<div data-testid=\"companionColumn-2\">\n<div class=\"css-53u6y8\">\n<p class=\"css-at9mc1 evys1bk0\">David V. Pomeranz, president and chief executive of RiverSpring Living \u2014 its campus includes independent and assisted living, memory care, rehab and a nursing home \u2014 sees such efforts as ways \u201cto restore normalcy to people who\u2019ve been stripped of so much.\u201d<\/p>\n<\/div>\n<aside class=\"css-ew4tgv\" aria-label=\"companion column\"\/><\/div>\n<div data-testid=\"companionColumn-3\">\n<div class=\"css-53u6y8\">\n<p class=\"css-at9mc1 evys1bk0\">Taking a group of residents with dementia to a real store might prove overstimulating, he said, and people couldn\u2019t simply leave when they\u2019d had enough. But an only-sort-of-real store or nursery \u201cgives them those life experiences which are familiar, which are comfortable, which are empowering and negate the feeling that they don\u2019t have control over their lives.\u201d<\/p>\n<p class=\"css-at9mc1 evys1bk0\">It\u2019s a strategy with advocates \u2014 and some critics.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">A few decades ago, those caring for people with dementia, whether at home or in facilities, took a very different approach.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">They tried \u201creality orientation,\u201d reminding patients that today is Tuesday, not Thursday. That they couldn\u2019t \u201cgo home\u201d because their house had been sold. That their spouses weren\u2019t visiting because they had died years ago (causing fresh shock and grief with every repetition).<\/p>\n<p class=\"css-at9mc1 evys1bk0\">\u201cIt didn\u2019t work,\u201d said Steven Zarit, emeritus professor at Penn State and a longtime researcher on caregiving and dementia. \u201cIt didn\u2019t help people\u2019s memories, it didn\u2019t help their adjustment, it wasn\u2019t useful.\u201d<\/p>\n<\/div>\n<aside class=\"css-ew4tgv\" aria-label=\"companion column\"\/><\/div>\n<div data-testid=\"companionColumn-4\">\n<div class=\"css-53u6y8\">\n<p class=\"css-at9mc1 evys1bk0\">Instead, caregivers have largely adopted a strategy, sometimes called \u201c<a class=\"css-yywogo\" href=\"https:\/\/bristoluniversitypressdigital.com\/view\/journals\/ijcc\/8\/1\/article-p178.xml\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">therapeutic lying<\/a>,\u201d that gently deflects painful questions. Where is a (deceased) loved one? \u201cI\u2019m sure he\u2019ll be here soon. You know how traffic is. Let\u2019s go for a walk while we wait.\u201d<\/p>\n<p class=\"css-at9mc1 evys1bk0\">The introduction of <a class=\"css-yywogo\" href=\"https:\/\/www.nytimes.com\/2020\/09\/26\/health\/coronavirus-elderly-isolation-robot-pets.html\" title=\"\">robotic pets<\/a> that purred and woofed, and baby dolls to care for, extended that approach. Especially when the pandemic restricted other kinds of interactions, some people with dementia seemed to enjoy such inanimate companions.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Creating whole environments, which may evoke the past or may simply allow people to feel they\u2019re participating in the present, appears to be the next step.<\/p>\n<\/div>\n<aside class=\"css-ew4tgv\" aria-label=\"companion column\"\/><\/div>\n<div data-testid=\"companionColumn-5\">\n<div class=\"css-53u6y8\">\n<p class=\"css-at9mc1 evys1bk0\">In 2018, the nonprofit Glenner Alzheimer\u2019s Family Centers developed the <a class=\"css-yywogo\" href=\"https:\/\/glenner.org\/town-square\/town-square-faq\/\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">Town Square adult day program<\/a>, replicating a small-town Main Street of the 1950s within a large warehouse in Chula Vista, Calif.<\/p>\n<\/div>\n<aside class=\"css-ew4tgv\" aria-label=\"companion column\"\/><\/div>\n<div data-testid=\"companionColumn-6\">\n<div class=\"css-53u6y8\">\n<p class=\"css-at9mc1 evys1bk0\">It features a retro diner for meals, a library displaying Ike\u2019s portrait, a space mimicking a vintage movie theater and atmospheric touches like a 1959 Thunderbird and an old-fashioned phone booth. Franchisees have opened nine similar Town Squares in seven states, with more in development.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Day programs have demonstrated benefits for cognitively impaired participants and their caregivers, but \u201cthis environment allows us to go deeper into reminiscence therapy,\u201d said Lisa Tyburski, chief marketing officer for Glenner, referring to the use of prompts and objects to encourage memories and communication.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">For participants, \u201cit brings so much peace to be able to have a conversation about something they recall,\u201d Ms. Tyburski said. \u201cWe see them laughing and smiling, forming friendships.\u201d<\/p>\n<p class=\"css-at9mc1 evys1bk0\">There\u2019s <a class=\"css-yywogo\" href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S152586102300316X\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">scant evidence<\/a> that such environments, including <a class=\"css-yywogo\" href=\"https:\/\/hogeweyk.dementiavillage.com\/\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">dementia villages<\/a> in Europe that create <a class=\"css-yywogo\" href=\"https:\/\/journals.sagepub.com\/doi\/10.1177\/1471301218820480\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">entire residential neighborhoods<\/a> (but don\u2019t mimic the past), provide clinical benefits or reliably improve quality of life.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Yet \u201cenvironment is really important, and it can be enabling or disabling,\u201d said Andrew Clark, co-editor of the book \u201cDementia and Place\u201d and a professor at the University of Greenwich in England.<\/p>\n<\/div>\n<aside class=\"css-ew4tgv\" aria-label=\"companion column\"\/><\/div>\n<div data-testid=\"companionColumn-7\">\n<div class=\"css-53u6y8\">\n<p class=\"css-at9mc1 evys1bk0\">\u201cWe need to find ways for people to connect, to maintain routines and everyday activities,\u201d he said. Such environments may encourage those with dementia \u201cto engage with people, to get out and about, to not be shut away.\u201d<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Some experts express ambivalence and ethical concerns. Dr. Clark supports the shift from reality orientation. \u201cIn dementia, there are all sorts of situations where not telling the truth could be better for people\u2019s well-being,\u201d he said.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">But the ethics get \u201cmurky,\u201d he added, if well-intentioned caregivers treat people with dementia like children. To Dr. Zarit, for instance, distributing baby dolls \u201cfeels infantilizing.\u201d<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Throwback Main Streets \u201ctest the limits of how much is this creativity versus deception,\u201d said Dr. Jason Karlawish, a geriatrician and co-director of the Penn Memory Center. \u201cIt starts to become problematic if it \u2018others\u2019 people,\u201d he said, creating distance between those with cognitive impairment and everyone else.<\/p>\n<\/div>\n<aside class=\"css-ew4tgv\" aria-label=\"companion column\"\/><\/div>\n<div data-testid=\"companionColumn-8\">\n<div class=\"css-53u6y8\">\n<p class=\"css-at9mc1 evys1bk0\">\u201cI think we could find more creative ways to engage in meaningful activities,\u201d he added.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Indeed, dementia programs around the country increasingly have offerings like <a class=\"css-yywogo\" href=\"https:\/\/www.anne-basting.com\/creative-care-1\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">interactive theatrical experiences<\/a>, <a class=\"css-yywogo\" href=\"http:\/\/www.momentiaseattle.org\/\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">opportunities<\/a> to make art and explore music, efforts to <a class=\"css-yywogo\" href=\"https:\/\/alterdementia.com\/services\/\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">connect through church congregations<\/a>, intergenerational gatherings with real children, and pet therapy with live animals. Hundreds of <a class=\"css-yywogo\" href=\"http:\/\/dfamerica.org\/what-is-memory-cafe\/\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">Memory Cafes<\/a> meet regularly.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Nancy Berlinger, an ethicist and <a class=\"css-yywogo\" href=\"https:\/\/onlinelibrary.wiley.com\/doi\/10.1002\/hast.1549\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">researcher at the Hastings Center<\/a>, points to another concern about dementia-focused environments: \u201cSo much of this comes down to what you can afford.\u201d<\/p>\n<p class=\"css-at9mc1 evys1bk0\">At franchised Town Squares, participants pay an average of $150 a day. (Medicaid, Veterans Affairs and state and local agencies sometimes subsidize day care costs.) At RiverSpring, which already offers a full schedule of interactive programs, memory care costs $15,000 a month.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">(In New York City, for comparison, assisted living averaged $6,500 a month and nursing home care about twice that in 2023, according to <a class=\"css-yywogo\" href=\"https:\/\/www.genworth.com\/aging-and-you\/finances\/cost-of-care\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">Genworth\u2019s annual survey<\/a>.)<\/p>\n<p class=\"css-at9mc1 evys1bk0\">With dementia villages and environments, \u201cthe worry is that they become enclaves for the wealthy,\u201d Dr. Clark said.<\/p>\n<\/div>\n<aside class=\"css-ew4tgv\" aria-label=\"companion column\"\/><\/div>\n<div data-testid=\"companionColumn-9\">\n<div class=\"css-53u6y8\">\n<p class=\"css-at9mc1 evys1bk0\">Or that they become substitutes for adequate staffing. Creating RiverSpring\u2019s nursery and store was inexpensive, Mr. Pomeranz said. But staffing isn\u2019t, and to function as intended, the environments require employees engaging in extended conversations.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Many nursing homes and assisted living facilities, perennially short-staffed, struggle to respond to basic needs like escorting residents to the bathroom, let alone facilitating shopping at a twice-weekly store. Instead of hiring and training enough people, administrators may be tempted to simply pass around dolls and robo-pets.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Nevertheless, the continuing search for ways to make life more stimulating and sustaining for elders with dementia, a <a class=\"css-yywogo\" href=\"https:\/\/www.nytimes.com\/2025\/01\/13\/health\/dementia-cases-us.html\" title=\"\">growing proportion of the population<\/a>, wins applause all around.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">\u201cThe choice to restore their brains to an undamaged state does not exist,\u201d Dr. Berlinger said.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">But caregivers can \u201ctry to meet people where they are and say: \u2018What gives comfort? What reduces stress?
