In a remarkable departure from the standard for medical evidence reviews, the authors were not identified pending a post-publication review process that would begin in \u201cthe coming days.\u201d<\/p>\n<p class=\"css-at9mc1 evys1bk0\">H.H.S. officials declined to answer questions about what the review process would entail. The administration has threatened to <a class=\"css-yywogo\" href=\"https:\/\/www.nytimes.com\/2025\/02\/14\/us\/seattle-judge-trump-trans-youth-hospitals.html\" title=\"\">end funding<\/a> for hospitals that provide gender-transition treatments to people under 19 and to bar openly transgender people from <a class=\"css-yywogo\" href=\"https:\/\/www.nytimes.com\/2025\/04\/24\/us\/politics\/trump-supreme-transgender-troops.html\" title=\"\">serving in the military<\/a>.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">The administration has moved to house transgender women <a class=\"css-yywogo\" href=\"https:\/\/www.nytimes.com\/2025\/02\/21\/us\/trump-trans-women-prison.html\" title=\"\">in federal prisons with men<\/a> and to no longer reflect the gender identities of transgender people on <a class=\"css-yywogo\" href=\"https:\/\/www.nytimes.com\/2025\/04\/18\/us\/trump-passport-changes-transgender.html\" title=\"\">passports<\/a>.<\/p>\n<\/div>\n<aside class=\"css-ew4tgv\" aria-label=\"companion column\"\/><\/div>\n<div data-testid=\"companionColumn-4\">\n<div class=\"css-53u6y8\">\n<p class=\"css-at9mc1 evys1bk0\">\u201cIs the administration\u2019s hostility to this medical care based on actual scientific research, or is it ideologically driven by this administration\u2019s open disapproval of transgender people and belief that transgender identity is false?\u201d said Shannon Minter, legal director at the National Center for Lesbian Rights.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">The center represents transgender individuals in several lawsuits challenging the constitutionality of the administration\u2019s policies.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Casey Pick, director of law and policy at the Trevor Project, a nonprofit organization focused on suicide prevention among L.G.B.T.Q. young people, said, \u201cTo the extent that this is a document that is challenging even the ability of a young person to identify as transgender, that is an ideological statement \u2014 that is not a medical statement.\u201d<\/p>\n<p class=\"css-at9mc1 evys1bk0\">The report is a victory for those who have described this field of medicine as part of a broader project that <a class=\"css-yywogo\" href=\"https:\/\/www.heritage.org\/gender\/commentary\/what-gender-ideology\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">denies the realities<\/a> of <a class=\"css-yywogo\" href=\"https:\/\/lgbcouragecoalition.substack.com\/p\/lgb-courage-coalition-applauds-hhs\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">biological sex<\/a>.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Roger Severino, vice president for domestic policy at the Heritage Foundation, a conservative think thank, applauded the H.H.S. report and blamed \u201cprofit-seeking doctors and ideological groups\u201d for persuading families that \u201ca child\u2019s sex is whatever they say it is.\u201d<\/p>\n<\/div>\n<aside class=\"css-ew4tgv\" aria-label=\"companion column\"\/><\/div>\n<div data-testid=\"companionColumn-5\">\n<div class=\"css-53u6y8\">\n<p class=\"css-at9mc1 evys1bk0\">About 3 percent of high school students identify as transgender, according to <a class=\"css-yywogo\" href=\"https:\/\/www.nytimes.com\/2024\/10\/08\/health\/transgender-teenagers-cdc-survey.html\" title=\"\">government data<\/a>, a number that has <a class=\"css-yywogo\" href=\"https:\/\/www.cdc.gov\/mmwr\/volumes\/68\/wr\/mm6803a3.htm\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">risen<\/a> substantially over the last decade. Mr. Trump made it a focus of his campaign, and some Democrats believe the strategy helped him win.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">The new H.H.S. report goes further than similar reports in <a class=\"css-yywogo\" href=\"https:\/\/www.nytimes.com\/2024\/04\/09\/health\/europe-transgender-youth-hormone-treatments.html\" title=\"\">European countries<\/a> that have introduced new restrictions on gender treatments for adolescents.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">An independent <a class=\"css-yywogo\" href=\"https:\/\/webarchive.nationalarchives.gov.uk\/ukgwa\/20250310143933\/https:\/\/cass.independent-review.uk\/home\/publications\/final-report\/\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">review of clinical services<\/a> for young people in England, led by Dr. Hilary Cass, former president of the Royal College of Pediatricians, reached similar conclusions about the <a class=\"css-yywogo\" href=\"https:\/\/adc.bmj.com\/pages\/gender-identity-service-series\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">quality of evidence<\/a> to support the use of puberty blocking and hormonal treatments in minors. (Surgeries are not available for minors in Britain.)<\/p>\n<p class=\"css-at9mc1 evys1bk0\">But that review, conducted over a four-year period, interviewed young patients, transgender adults, parents and clinicians to gain a broad picture of the medical field.<\/p>\n<\/div>\n<aside class=\"css-ew4tgv\" aria-label=\"companion column\"\/><\/div>\n<div data-testid=\"companionColumn-6\">\n<div class=\"css-53u6y8\">\n<p class=\"css-at9mc1 evys1bk0\">Dr. Cass concluded that the evidence on both the benefits and the risks of the treatments was \u201cremarkably weak\u201d but said that some young people were still likely to benefit from early intervention.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">\u201cThere are young people who absolutely benefit from a medical pathway, and we need to make sure that those young people have access \u2014 under a research protocol, because we need to improve the research \u2014 but not assume that that\u2019s the right pathway for everyone,\u201d Dr. Cass <a class=\"css-yywogo\" href=\"https:\/\/www.nytimes.com\/2024\/05\/13\/health\/hilary-cass-transgender-youth-puberty-blockers.html\" title=\"\">said in an interview<\/a> last year.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">That review <a class=\"css-yywogo\" href=\"https:\/\/webarchive.nationalarchives.gov.uk\/ukgwa\/20250310143633\/https:\/\/cass.independent-review.uk\/home\/publications\/final-report\/\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">concluded<\/a> by recommending that England\u2019s National Health Service restrict the use of puberty blocking drugs to research trials only, and clinicians were advised to prescribe hormones to teenagers with \u201cextreme caution.\u201d<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Other clinicians who have voiced concerns about the field of youth gender medicine were ambivalent about how the new report might be used.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">\u201cI am glad that authorities in the United States are finally taking into account what\u2019s been going on in Europe in recent years,\u201d said Erica Anderson, a child psychologist and former president of the U.S. Professional Association of Transgender Health.<\/p>\n<\/div>\n<aside class=\"css-ew4tgv\" aria-label=\"companion column\"\/><\/div>\n<div data-testid=\"companionColumn-7\">\n<div class=\"css-53u6y8\">\n<p class=\"css-at9mc1 evys1bk0\">She has been <a class=\"css-yywogo\" href=\"https:\/\/www.washingtonpost.com\/outlook\/2021\/11\/24\/trans-kids-therapy-psychologist\/\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">vocal<\/a> about her concerns that youth gender clinics have moved away from careful mental health assessments as the population of children seeking gender treatments has grown.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">But Dr. Anderson, who still <a class=\"css-yywogo\" href=\"https:\/\/www.supremecourt.gov\/DocketPDF\/23\/23-477\/323992\/20240903172206889_23-477tsacDrEricaEAndersonPhD.pdf\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">supports<\/a> early intervention for some young people, noted that it was impossible to ignore the inflammatory executive order that led to the report.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">\u201cIt\u2019s kind of like calling out to someone a rank insult and then deciding you want to have a conversation with them,\u201d she said. \u201cWell, that doesn\u2019t really work very well with actual human beings with feelings and histories.\u201d<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Mr. Minter, of the National Center for Lesbian Rights, said that by emphasizing psychotherapy over medical interventions, the H.H.S. report effectively endorsed conversion therapy, intended to change a minor\u2019s gender identity or sexual orientation.<\/p>\n<p class=\"css-at9mc1 evys1bk0\"><a class=\"css-yywogo\" href=\"https:\/\/www.ama-assn.org\/system\/files\/conversion-therapy-issue-brief.pdf\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">Leading<\/a> <a class=\"css-yywogo\" href=\"https:\/\/www.aacap.org\/aacap\/Policy_Statements\/2018\/Conversion_Therapy.aspx\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">medical<\/a> <a class=\"css-yywogo\" href=\"https:\/\/www.apa.org\/topics\/lgbtq\/sexual-orientation-change\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">groups<\/a> have supported bans on the practice, citing evidence that it causes depression, anxiety and self-hatred.<\/p>\n<\/div>\n<aside class=\"css-ew4tgv\" aria-label=\"companion column\"\/><\/div>\n<div data-testid=\"companionColumn-8\">\n<div class=\"css-53u6y8\">\n<p class=\"css-at9mc1 evys1bk0\">But the Supreme Court has agreed to hear a First Amendment <a class=\"css-yywogo\" href=\"https:\/\/www.nytimes.com\/2025\/03\/10\/us\/politics\/supreme-court-conversion-therapy.html\" title=\"\">challenge to a Colorado ban on conversion therapy<\/a> next term, brought by a licensed professional counselor whose Christian faith includes the belief that \u201cpeople flourish when they live consistently with God\u2019s design, including their biological sex,\u201d according to court documents.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Other therapists, including Dr. Anderson, have argued that so-called <a class=\"css-yywogo\" href=\"https:\/\/www.washingtonpost.com\/outlook\/2021\/11\/24\/trans-kids-therapy-psychologist\/\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">\u201cexploratory\u201d therapy<\/a> with supportive clinicians can be helpful for helping to disentangle mental health issues from gender identity for adolescents.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Kellan Baker, who studies transgender health policy at Whitman Walker, a nonprofit community health center in Washington, said the report was a departure from how health policy had typically been shaped in the United States.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">\u201cIt is important that medicine be practiced by those with expertise in it, by trained clinicians operating according to standards of care that are set out by their respective medical fields \u2014 not by the federal government,\u201d Dr. Baker said.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">He said he worried that the report might be cited by the government as justification for refusing to cover medical care for transgender young people.<\/p>\n<\/div>\n<aside class=\"css-ew4tgv\" aria-label=\"companion column\"\/><\/div>\n<div data-testid=\"companionColumn-9\">\n<div class=\"css-53u6y8\">\n<p class=\"css-at9mc1 evys1bk0\">The Centers for Medicare and Medicaid Services, a division of H.H.S., <a class=\"css-yywogo\" href=\"https:\/\/www.cms.gov\/files\/document\/letter-stm.pdf\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">sent a letter<\/a> to state Medicaid agencies last month urging them not use Medicaid funds for gender-transition care for minors.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Pam Bondi, the attorney general, has directed the Justice Department to <a class=\"css-yywogo\" href=\"https:\/\/s3.documentcloud.org\/documents\/25912589\/bondi-memo-42225.pdf\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">investigate doctors<\/a> who provide such care.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">\u201cThis document is not a standard of care, but it\u2019s going to be cited by the government as justification for refusing to cover medical care for transgender young
