The United States has just lost its ability to detect it.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Among the Centers for Disease Control and Prevention employees fired on Tuesday were 77 scientists who, among other work, gathered samples of gonorrhea and other S.T.I.s from labs nationwide, analyzed the genetic information for signs of drug resistance, and readied the samples for storage at a secure facility.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">No other researchers at the agency have the expertise, or the software, to continue this work. The abrupt halt has stranded about 1,000 samples of gonorrhea and other sexually transmitted pathogens that had not yet been processed, and perhaps dozens more headed to the agency.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">There are as many as 30 freezers full of samples that now have no custodians, said one senior C.D.C. official who spoke on condition of anonymity for fear of retaliation.<\/p>\n<\/div>\n<aside class=\"css-ew4tgv\" aria-label=\"companion column\"\/><\/div>\n<div data-testid=\"companionColumn-1\">\n<div class=\"css-53u6y8\">\n<p class=\"css-at9mc1 evys1bk0\">\u201cWe were just really shut down midair, like there was no warning,\u201d the official said. \u201cIt was just completely unplanned and chaotic.\u201d<\/p>\n<p class=\"css-at9mc1 evys1bk0\">The C.D.C.\u2019s work on S.T.I.s had taken on greater urgency in the past few years as rates of new infections soared. More than 2.4 million new S.T.I.s were diagnosed in 2023, about <a class=\"css-yywogo\" href=\"https:\/\/www.cdc.gov\/std\/stats\/archive\/2004SurveillanceAll.pdf#page=108.31\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">one million more<\/a> than 20 years ago.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Nearly 4,000 babies were born with congenital syphilis in 2023. About 280 were stillborn or died soon after.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">\u201cWhoever got rid of the lab just doesn\u2019t understand how important the lab is,\u201d said another senior C.D.C. official who spoke on condition of anonymity.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">About 600,000 new gonorrhea cases <a class=\"css-yywogo\" href=\"https:\/\/www.cdc.gov\/sti-statistics\/data-vis\/table-gc-agesex.html\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">were diagnosed<\/a> in the United States in 2023. The bacteria that cause gonorrhea, called Neisseria gonorrhoeae,<em class=\"css-2fg4z9 e1gzwzxm0\"> <\/em>spread through sexual contact to the genitals, rectum and throat. Left untreated, it can cause infertility and sterility, blindness in infants or even death.<\/p>\n<\/div>\n<aside class=\"css-ew4tgv\" aria-label=\"companion column\"\/><\/div>\n<div data-testid=\"companionColumn-2\">\n<div class=\"css-53u6y8\">\n<p class=\"css-at9mc1 evys1bk0\">Gonorrhea has <a class=\"css-yywogo\" href=\"https:\/\/www.cdc.gov\/gonorrhea\/hcp\/drug-resistant\/index.html\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">become resistant<\/a> to nearly every available antibiotic, leaving a single class that still snuffs it out. The most powerful defense combines a shot of ceftriaxone with azithromycin, but some evidence hints that gonorrhea is <a class=\"css-yywogo\" href=\"https:\/\/www.who.int\/news-room\/fact-sheets\/detail\/multi-drug-resistant-gonorrhoea\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">evolving to sidestep<\/a> even that treatment.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Over more than 25 years, the C.D.C. lab archived about 50,000 gonorrhea samples \u2014 the largest collection in the world \u2014 which allow scientists to track how the pathogen has changed over time. It\u2019s not clear what will happen to the samples.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">One new public health strategy makes it even more important for the nation to track gonorrhea, said Dr. Jenell Stewart, an infectious diseases physician at Hennepin Healthcare in Minneapolis.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">In a bid to combat resurgent syphilis and chlamydia, the C.D.C. recommended last year that gay and bisexual men and transgender women take doxycycline, a widely used antibiotic, within 72 hours of unprotected sex.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Cities like San Francisco and Seattle that had earlier endorsed the practice, called doxy-PEP, have already seen <a class=\"css-yywogo\" href=\"https:\/\/www.nytimes.com\/2024\/03\/04\/health\/doxycycline-syphilis-chlamydia.html\" title=\"\">drastic drops<\/a> in the rates of those infections.<\/p>\n<\/div>\n<aside class=\"css-ew4tgv\" aria-label=\"companion column\"\/><\/div>\n<div data-testid=\"companionColumn-3\">\n<div class=\"css-53u6y8\">\n<p class=\"css-at9mc1 evys1bk0\">But researchers are worried that widespread use of doxycycline might increase resistance to the entire class of antibiotics, called tetracyclines. A few <a class=\"css-yywogo\" href=\"https:\/\/academic.oup.com\/cid\/advance-article\/doi\/10.1093\/cid\/ciaf089\/8045759\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">studies<\/a> <a class=\"css-yywogo\" href=\"https:\/\/www.nature.com\/articles\/s41591-024-03274-2\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">suggest<\/a> there may be <a class=\"css-yywogo\" href=\"https:\/\/jamanetwork.com\/journals\/jamainternalmedicine\/fullarticle\/2828994\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">reason to<\/a> worry.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Harvard University researchers last month <a class=\"css-yywogo\" href=\"https:\/\/www.medrxiv.org\/content\/10.1101\/2025.03.24.25324420v1\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">analyzed more than 14,000 genetic sequences<\/a> generated by C.D.C. researchers and found that the proportion of gonorrhea bacteria<em class=\"css-2fg4z9 e1gzwzxm0\"> <\/em>with antibiotic resistance increased to more than 35 percent last year, from less than 10 percent in 2020.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">The federal scientists who produced that data and made it publicly available have all been fired. \u201cWithout public health money and infrastructure, I\u2019m not sure who if anyone will take up the torch to monitor gonorrhea resistance,\u201d Dr. Stewart said.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">\u201cThis is a huge loss,\u201d she added.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Dr. Stewart and a colleague spent two years preparing the protocol and an app to study doxy-PEP in cisgender women and monitor gonorrhea resistance.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">The study was supported by the Adolescent Medicine Trials Network, whose funding was slashed last month.<\/p>\n<\/div>\n<aside class=\"css-ew4tgv\" aria-label=\"companion column\"\/><\/div>\n<div data-testid=\"companionColumn-4\">\n<div class=\"css-53u6y8\">\n<p class=\"css-at9mc1 evys1bk0\">At least <a class=\"css-yywogo\" href=\"https:\/\/taggs.hhs.gov\/Content\/Data\/HHS_Grants_Terminated.pdf\" title=\"\" rel=\"noopener noreferrer\" target=\"_blank\">five other grants<\/a> to study doxy-PEP have been terminated, along with a variety of grants at the National Institutes of Health aimed at preventing S.T.I.s, including H.I.V.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Any lab can test for S.T.I.s, but commercial tests cannot determine whether gonorrhea will respond to available treatments. C.D.C. scientists developed the only such test, and provided funding and training to a few dozen labs on the sophisticated testing.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">Samples were sent to the agency for confirmation. Without the agency scientists, testing for drug sensitivity will most likely cease, several experts said.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">\u201cWe cannot have a national surveillance system without a national lab,\u201d said one scientist who leads a C.D.C.-funded lab but did not wish to be identified without authorization to speak to the media.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">C.D.C. scientists were also helping to develop alternatives to the nation\u2019s outdated syphilis test. It cannot identify an active infection, only whether someone was ever infected. The agency has three large contracts to develop new rapid syphilis tests.<\/p>\n<\/div>\n<aside class=\"css-ew4tgv\" aria-label=\"companion column\"\/><\/div>\n<div data-testid=\"companionColumn-5\">\n<div class=\"css-53u6y8\">\n<p class=\"css-at9mc1 evys1bk0\">But without expertise and the samples from C.D.C. scientists, it\u2019s unclear whether that work can continue, said a senior official with knowledge of the situation.<\/p>\n<p class=\"css-at9mc1 evys1bk0\">The fired scientists had about 1,400 years of field experience between them.
